Your hospital record

Medical records

The Medical Records Department issues new hospital cards, stores and retrieves case notes, codes and indexes diagnoses, registers deaths and prepares the hospital's statistical returns to the Federal Ministry of Health. Your record is the basis for planning your care, for communication between the professionals treating you, and for protecting the legal interests of patients, clinicians and the hospital.

  • New patients are registered and issued a card at the records desk
  • Returning patients present their card so case notes can be retrieved
  • Records also support teaching, research and quality-of-care review
Health insurance

National Health Insurance enrollees

UNTH is a primary care provider for its National Health Insurance enrollees, who are seen at the General Outpatient Department on the Ituku-Ozalla campus and at the Federal Secretariat Clinic, Independence Layout, Enugu.

  • Report to the insurance desk after collecting a serial number so cover can be verified
  • Verified enrollees do not pass through the revenue point
  • Return to the insurance desk after consultation to register prescriptions, laboratory requests and referral letters
  • A dedicated insurance pharmacy dispenses medicines for enrollees
