Raise a Concern or Complaint

If something about your care, treatment or the conduct of staff has troubled you, tell us. Concerns are handled separately from general feedback and are treated in confidence.

This form is not for emergencies
Do not use this form in an emergency. If the situation is urgent, call the hospital emergency line or go directly to Accident and Emergency at Ituku-Ozalla.
What happens next
  1. 1Your concern is recorded confidentially and given a reference number.
  2. 2It is reviewed by authorised hospital staff responsible for patient relations.
  3. 3Where you have given contact details, a member of staff will acknowledge your concern and explain the next steps.
  4. 4Findings and any action taken are recorded so that lessons can improve future care.
How your information is handled
The information you provide is transmitted securely and held confidentially by the University of Nigeria Teaching Hospital. It is used only to respond to your submission and is never published on this website, included in site search, or used to train artificial-intelligence systems. Please do not include detailed clinical or diagnostic information in this form.
Complaints are never published
Submitting this form sends your message privately to UNTH. It is not published on this website. Complaints, reports and feedback are reviewed by authorised hospital staff, and only a patient experience that you have consented to, and that a UNTH reviewer has edited or anonymised and approved, may ever appear publicly.
Routing of concerns
Awaiting UNTH confirmation: the administrative inbox or unit that should receive these submissions has not yet been confirmed. Until then, every submission is stored securely in the hospital's private records for review by authorised staff.
